Our Admin will email you a link to complete these forms online. Other than the Healthcare Provider letter and Client Medical History & Physician's Statement, forms here are for reference only. The Healthcare Provider letter and Client Medical History & Physician's Statement must be printed/downloaded and given to your health care provider to complete and sign. All must be completed before your first lesson. Information and signatures must also be updated annually.
